I have been a physiotherapist for 30 years and a clinic owner for six of these years. My work experience includes pediatrics, neurological rehabilitation, orthopedics, osteoporosis/arthritis and vestibular rehabilitation. My experience in pediatrics gave me the background in understanding the vestibular system, in particular in children with cerebral palsy, as they have limited movement. Treatment activities included moving the children on swings, therapy balls or scooters to help develop their sense of motion and balance.
Over the past sixteen years I have specialized in vestibular rehabilitation. I was looking for a way to provide an objective measure of balance, so that we don’t rely only on clinical results that can have a subjective bias through the use of semi-descriptive words (mild, moderate of severe sway) or by using time as a reference for balance. I began working with an old computerized posturography unit resurrected from storage in one of the hospitals.
Seven years ago I heard of a smaller and newer system, Tetrax and purchased one.
My father was one of the first people I assessed on Tetrax. At the time he was 82 and having difficulties with his balance and falling often, but did not have a diagnosis. The developer of Tetrax, Prof. Kohen-Raz, was able to interpret the finding, which identified a neurological condition. Based on this we sought further medical investigations and he was diagnosed with Parkinson’s Disease and was given medications which helped his balance.
To date, I’ve used Tetrax on close to 700 clients, mainly for diagnosis and baseline measurement. The assessment analysis shown in the reports indicates the sensory system(s) that are involved in the imbalance, if present. Tetrax is unique as it analyzes sway according to frequency of motion, and thereby can indicate if there is a visual, vestibular, somatosensory or neurological cause for the imbalance, as well as how the head/neck position can affect balance. In most cases, the Tetrax results match the clinical testing.
Tetrax is an excellent device to help discern if neck issues are contributing to the balance problem. In my experience in treating people with dizziness and balance concerns, the neck is a crucial area to be addressed. Tetrax is unique in it’s ability to test people while standing with the head bent forward, looking up or turned in either direction.
Tetrax can show how two or more sensory systems can be involved in the imbalance. The causes of imbalance and falls are usually multifactorial. For example, Tetrax can show if the vestibular system and neck and perhaps a neurological component are present.
Tetrax is a very sensitive device for measuring sway or imbalance. Very small corrections in balance by the client are detected which may not be detected when visually watching the balance. When re-evaluating balance on the same person at a later date, the sensitivity of Tetrax also allows for the comparison of improvement or regression to be seen even with small increments of change.
Tetrax is one of the best investments I’ve made for my clinic.

I met Prof. Kohen Raz for the first time fifteen years ago. I saw a publication in Percept. Mot. Skills Journal* and contacted him. He happened to be coming to Milan, we met and I was introduced to Tetrax.
What was so compelling for me was the innovative concept he presented:
Our brain is “movement ready”. Even when we’re completely steady and motionless it is wired for movement. This is the underlying concept that enables Tetrax to measure the ability and quality of movement even when a person is standing still. So it is not necessary to observe a subject’s movement. You can measure their stability through Tetrax and analyze the findings to predict how the person will perform.
This is one of the reasons Tetrax findings are so precise with air force pilot cadets as well as predicting fatigue and quality of performance of pilots. ** (get research quote here)
This very same concept has just been described in a new book by the French physiologist Alain Berthoz “Simplexity”.
Tetrax embodies Simplexity: simultaneously simple and complex.
It operates on two distinct and separate levels:
It can be used in a simple way for routine examinations,
AND it can also provide a vehicle for sophisticated and complex research.
There are many systems and ways to measure and observe the instability of a patient. But only Tetrax can show you the HOW – what is the character of the imbalance?
When we quietly stand in an upright position, we sway. Sway is apparently random but mathematical analysis of this sway (called Fourier Frequency Transformation) shows that sway is like a painting: the final observable aspect of sway is a combination of different colors (different frequencies).
Every patient is like a painting. Tetrax is able to measure the combination of these “colors” and thus enables the clinician to understand if the “painting” is correct (harmonic) or not and determine if and how to modify some aspects of this painting.
In this way a clinician can address rehabilitation in a very precise way and is able to follow-up improvements of patient balance even before general stability improves.
My specialty is vertigo and balance disorders. I use Tetrax every day as part of my lab equipment for diagnosis of various disorders, as well as for diagnosis and rehabilitation of Multiple Sclerosis, and for research.
In my research with hockey players, we compared professional and amateur players, both groups being very strong and with good balance. But only Tetrax uncovered that the quality of balance in professional players was markedly different and better than the balance of the amateur players.
Similarly, comparing young ice skaters with other athletic girls: They were all stable and strong, but Tetrax showed a distinct difference in weight distribution – a quality of balance that was specific, completely different and better in the ice skaters.
In MS rehabilitation we have extensive data (as of yet, unpublished) showing a modification of distribution frequency: the Tetrax rehabilitation system can help improve the ability of patients to maintain an upright position using less energy and thus minimizing fatigue. We are currently using Tetrax to collect data on dyslexic children – there is a clear connection between dyslexia and balance.
These are just a few small examples. I use Tetrax everyday in my clinical activities, but I am well aware that I’m using only 50% of the power and potential of this system. I wish I had more time to research and understand all of it.
Tetrax is ahead of it’s time, too modern for doctors like us; it will a job for the next generation of doctors to uncover it’s full potential. It is the instrument of the future.

For more than 13 years I have been routinely using the Tetrax System with it’s attached documentation in my ENT clinic for the purpose of the differential diagnosis of vertigo.
To this day, we have gathered experience with the measurements of more than 4000 patients. We mainly apply the Tetrax System to define and evaluate mixed types of vertiginous pathology, to differentiate between peripheral and central sources of vertigo, as well as to follow up the patient’s status after episodes of vertiginous attacks.
Unlike other traditional posture graphic systems, the Tetrax System (need to change to new name here) is based on measurements of the interaction of vertical pressure fluctuations over four independent platforms. The evaluation includes a differential Fourier analysis of five important frequency bands of postural sway. The evaluated measurements, which reflect the typical pattern of the pathology, are repeatedly found in later follow up control assessments.
Of special value are the Tetrax assessments in the context of the longitudinal evaluation of the progress of vertiginous states. Here the scope of assessment extends from the diagnosis of vertigo after cranial and cerebral injuries and cervical Whiplash trauma to the exploration of Parkinson’s Disease.
The Tetrax System has a peculiar power in supplying evidence in the context of legal expertise. In such situations, powerful and reliable arguments are supplied to define the degree of reduction or complete loss of working capacity. The relevant courts of social justice have fully recognized the procedure. Recently an important aspect has been added from the point of view of the (German) labor legislation. According to new mandatory labor-medical check ups, jobs defined G – 41, which involve danger of falling, require an assessment of postural control. In such cases, the Tetrax System is able to yield reliable information regarding the ability to tolerate heights, and the level of falling risk.
